Beliefs determine how we see, interact and feel the world around us. Beliefs are ideas that are formed after repetition and contemplation that are accepted as truth and reality. Therefore they impact how we see life.

Beliefs and knowledge are often in conflict. The difference might be subtle, but significant. Knowledge is something that you and others, who might be considered experts in that field, consider to be true and there is reasonable, plausible, and provable scientific explanation for that knowledge.

A belief on the other hand is based on personal experience or pure faith.

Too many people require you to accept their belief based on blind faith. Belief is not a fact and therefore usually not provable beyond a shadow of a down.

Recall that it wasn\’t long ago that people believed the world is flat, and that we thought women were witches, hunted them down, and burned them.

Welcome to belief, which is based on superstitious people, or ignorance.

Ignorant humans once believed that the Earth was the center of the universe, and that it was flat. Really they demonstrated was that what really was FLAT was the human ability to think outside the box and perceive something that others could not perceive.

When scientists discovered great things, hundreds of years ago, they were imprisoned, tortured and murdered. The Catholic Church did not want people to see beyond, and therefore stopped the voices of these researchers.

Actually, during that time in history it was thought to be common knowledge that these ancient flat world beliefs were true. Even though humankind was sincere in its ignorant beliefs, those beliefs were not true and by not believing as the others told you to, you were subject to arrest, imprisonment, and torture which often lead to death.

Times have changed. Today, things that you believe have changed, and you may believe in something so strongly that you count it as knowledge. When, in fact, there may be considerable proof from the past experience that a particular belief is a truth – its only truth for you from your personal experience. Just because you believe it is true, does not make it true. Although it might become a truth for you personally, by believing it you are limiting yourself by holding on to self-limiting beliefs.

The trick is to pull back the veil and see the real truth. That is one that is universal and does not need to be believed, it just is. These are referred to as universal truths. They exist and work beyond our own self-limiting beliefs. They forge and work through the universe and in our lives.

Gravity is a truth. Believe in it or not. Its still a real truth. If you drop something it will fall to the ground. If you trip, and fall, you could hurt your butt. You can only fall DOWN, you don\’t fall up.

Now this is only a truth that exists here on Earth under normal circumstances. There are other forces that can resist this law such as a strong wind. You may have been programmed since childhood to believe things that you thought to be true – but, once you realized that they were not true, you may have wondered why you wasted time and energy believing in them at all.

Take the case of Santa Claus. You believed he was real. You believed that by some mystical miracle he went around the world in one night and brought presents to every well-behaved child. You may have further believed that he ate the burnt cookies you left for him and he drank that horrible lactose free milk. You believe that it was all associated to your doing good things and not bad things, and that Santa was, well, almost like God!

There was strong evidence to your belief:

Your parents persuaded that you he was real. (You trusted your parents unquestionably)

The cookies were disappeared and the milk glass was empty. Even the crumbs disappeared.

There were presents under the Christmas tree by the morning.

You saw Santa at the supermarket, and you had a picture of Santa to prove it.

Your cousins believed in him too.

All of this evidence pointed toward knowledge, you just knew he existed. It pointed to a truth in that not only did you know he existed, everyone around you knew also. This belief as you found out was not true. It had shaped your view of the world as a child and still may have a lingering impact on your current belief system.

The point is that there are other things that you may think are true, but are not. You have been told things all your life for a variety of reasons: to make you feel better, to be funny, to hurt you, because other people believed it was true, and for other noble and not so noble reasons. You made the choice whether to believe what was being told you. You weighed this information and decided whether it fit or not into your already existing belief system. You either rejected the idea straight out, accepted it as part of what you already believed, or it changed your belief system.

Beliefs are very powerful, and they do shape our reality. If you believe in something strong enough, it becomes real in your life. There is a universal law that states that likes attract like. If you believe in something strong enough it not only becomes your reality, it attracts similar things in your life.

Suppose you believe you are clumsy. Your parents said you were clumsy, your coach as school did and so did your friends. They told you that you were an accident waiting to happen. You thought this to be true. Your whole life you tripped over your shadow. Not only did you believe this, but you also created the reality and circumstances of being clumsy.

Examine your thoughts and draw back the veil. Well, Ill be. They seem to have repeatedly hypnotized me to trip myself up — all my life\” well, I\’ll be! And, then \’you don\’t have to be!\’ It\’s all your belief.

In the movie, he hears the mantra, \”If you build it, they will come.\” You may not know it, but you have your own \”field of dreams.\” It\’s your subconscious mind. With your subconscious mind, if you \”think\” it, it will come. What will come? Whatever you want, truly.

You have the power to create the life you want, to change who you are, to change whatever you think is missing or inadequate into something perfect, all through the use of your subconscious mind. It\’s truly a powerful and beautiful thing, and can heal, transcend current circumstances, make you feel good; or make you feel bad. The subconscious mind, truly, has the power to completely change our world if we take control of it.

If you\’re skeptical, you\’re not alone. However, give me a chance to show you with a few examples just how powerful your mind is. One of my favorite examples is actually from the medical field, and works on a scientific basis. It\’s been documented time and time again, actually, and it\’s called the \”placebo effect.\” The medical field has coined the term \”placebo effect\” to denote a specific type of effect. Oftentimes, for example, this term is used during medical trials. The control group is given a \”placebo,\” which is an inert substance that has no medical benefit whatsoever as a drug might give. It\’s used as a counterpoint to the other group, which gets the actual medication. This is to clearly prove any benefit (or detriment) the drug itself might give. However, both groups are told what the medication is for and what it should do for them, and neither group knows which pill they\’ve gotten; placebo or actual medication.

Most studies have shown at least some of what is called the \”placebo effect.\” That is, at least some of the people in the placebo group have shown \”effects\” from the \”drug,\” even though they\’ve actually gotten a placebo. That is, their subconscious minds decided that they had gotten the drug and produced some of the same benefits and side effects as they were told the actual medication would give them. Simply, these patients believed they had gotten the medication, so their subconscious minds produced the medication\’s effects to at least some degree.

Think about this in your own situation. Perhaps you haven\’t been involved in any medical trials, but when you go to the doctor, are you told a specific medication is going to give you side effects? If the doctor minimizes side effects to you, don\’t be surprised. A doctor knows how powerful one\’s mind is, and may actually think telling you will cause side effects; through the power of your subconscious mind. Therefore, a doctor may minimize these negative things, in the hopes that you won\’t experience them. Suggesting them, in other words, may increase your chances of having side effects. And of course, they also want you to be compliant with taking the medication, so that\’s another reason to minimize the negative effects of the drug.

The power of suggestion combined with subconscious mind power equals incredibly powerful results. If this works in the medical realm, why can\’t it work in the everyday realm of your life? What is preventing you from tapping into your mind power and positively changing your life? Some people use the power of suggestion to change the way their subconscious mind works.

Others though use visualization to alter the way their mind sees things. Visualization is simple, and it works. Visualization can be done a couple of ways. First you close your eyes and visualize what it is you want, actually see it as if you are looking at a picture.

Another way, and this is often used in conjunction with the first technique, is to create a physical picture. Let\’s say that what you want is a new car, find a picture of the car you want better yet, go get a picture taken of you with the car. Hang that picture up where you can see it every day; the more you see it, the more the vision is imprinted in your mind your subconscious mind the more power it has. The subconscious mind power that you have will make that vision a reality.

Although this has been common knowledge for years to the really rich and high achievers, it has only been recently that scientists have started to understand the very real scientific process associated with this.

A scientific field called quantum physics has shown that matter is no longer solid at the subatomic level and simply becomes pure energy. And, quantum physics has shown that the mind can actually manipulate this energy. In fact, quantum physics has actually proven that the energy itself does not exist UNLESS it is observed.

What does that mean? Matter or energy exists because it is observed, and for no other reason. It is the mind of the observer that makes the matter exist, and not the other way around. In short, matter; and reality itself; is actually a creation of the mind. This is truly \”subconscious mind power\” at its essence.

In fact this is what the ancient religions have been saying for many thousands of years and with new understandings in quantum physics, modern science is at last beginning to catch up. Whatever your spiritual belief may be, there is no denying that the science of quantum physics works.

Amputation of the Lower Limb

Amputation of a limb is a major event for an individual and can result in significant psychological difficulties as they undergo rehabilitation, get fitted for a prosthetic limb and learn to walk effectively again. The surgical planning has to be done with the aim that the patient can easily and comfortably wear the prosthesis, engage fully and speedily in rehabilitation and have a reduced level of energy expenditure when they are walking. Patients in this situation have a lot to learn – to get the prosthesis on and off, to check the skin for pressure areas, to walk on surfaces of varying difficulty and to manage the times when the prosthesis is off.

To manage all these skills and learn how to be as independent as possible the patients need a skilled team to manage them which includes their own doctor, the surgeon, a physiotherapist, an occupational therapist a prosthetist and perhaps an employment adviser. The number of lower limb amputations is likely to continue to rise as the elderly populations increase in more advanced industrialised countries, with ischaemic vessel disease the primary cause. The proportion of above knee to below knee amputations has changed as surgeons became more skilled at preserving the knee joint so that the present ratio is 30% above knee to 70% below knee.

Peripheral vascular disease (PVD) is the most common reason for amputation with a significant number of patients suffering an amputation on the other side within three years. Most patients are elderly and have ischaemic problems which are secondary to diabetes, with peripheral neuropathy a common difficulty which can lead to ulcers and gangrenous changes. Trauma to the lower limb which involves the arteries and nerves can be treated but may result in a leg which is painful and does not function well, meaning that an amputation would be preferable for speedy rehabilitation and return to normality.

Amputation is also employed for less common conditions such as infections, congenital lower leg abnormalities and tumours. The planning for an amputation should be viewed as an operation targeted at reconstruction and not just removing a body part, aiming for the planned independence and function of the patient. As the level of the amputation progresses up the leg this increases the work of walking, requiring increased levels of oxygen concentration, increased expenditure of energy levels and reducing the speed the person is able to walk. Below knee amputation shows little increase in energy needed for walking but mid thigh can increase this by fifty percent.

The energy requirements for gait are extremely important as amputated patients frequently suffer from ischaemic tissue problems or other medical conditions which lead to walking consuming much of their energy abilities. Independence in functional activities may be hard to achieve as much of their limited energy supplies is taken up with simply walking. After the amputation, due to the skin viability and ischaemic diagnosis, healing may be delayed and this can have an important bearing on the eventual outcome for the patient’s independence. The soft tissues at the site of amputation must act as the connecting point between the leg and the prosthesis.

To be able to walk effectively the soft tissues of the amputation stump must be large and of good enough quality to cope with the longitudinal and shearing stresses which will occur in its relationship with the socket of the prosthesis. The end of the limb can directly bear weight in a longitudinal manner in some amputations such as through the ankle or through the knee, although these through joint amputations could be difficult. The new joint in the knee in this case is below the original and this makes the knee stick out further than the original and the calf region much shorter than normal.

PVD or peripheral vascular disease is the most prevalent reason for amputation, with elderly patients being the largest group and often having a second amputation inside three years. Ischaemia of the tissues occurs, often the result of diabetes which can then progress to neuropathy of the peripheral nerves, ulcers and eventually gangrenous changes in the limb. An accident to the leg involving open fractures and arterial and nerve damage can now be medically managed to save the leg but this may not always be positive as an amputation could permit early progress via rehabilitation towards independence.

Lower Limb Amputation – Part Two

Diagnosis is relatively straightforward as most patients will have peripheral vascular disease and will have had considerable treatment for it already. As the small blood vessels block off gradually the toes can develop gangrene and ulcers on the pressure areas, which permits bacterial invasion leading eventually to bony infection. If treatment is incremental there may be a long period of minor amputations and other operations, all the time the patient being unable to walk due to pain in the limb. They may use wheelchair due to the pain and energy requirement of ambulation.

In cases where the amputation is due to injury or accident it is called traumatic amputation and may be related to severe fractures which are compound and the leg has badly damaged arteries and nerves. The best choice here might be a primary amputation versus attempted saving of the leg. Salvage attempts for the leg can result in the leg being painful and poor in function, limiting progress and encouraging depression. Once the injury has occurred a realistic assessment needs to be made as to the potential for the limb to function well and to decide to amputate should that lead to faster functional return and pain relief.

With amputation the major goal is to preserve the length of the leg and to get the maximum functional use from the limb. As the operation cannot be reversed the surgeon must be sure the decision is correct and the only limits are the medical fitness of the patient to manage operation. Because the patient’s poor medical status will likely be closely linked to their abnormal limb the operation should remove the negative influence on their health and may save their life. Preparation for the operation, rehabilitation and life after amputation is a multidisciplinary matter also involving physiotherapists, social workers and psychologists.

Surgical management of severe leg trauma has shown significant advances in the ability to perform microsurgery to the vascular structures, advanced fixation of fractures and techniques to promote revascularisation of tissues. Amputation may then be viewed as a failure if these techniques cannot save the limb, but viewing it as a reconstructive process is more positive, allowing an increase in useful functional capacity. Techniques of amputation have seen much less development and patients still consult with difficulties such as persisting pain, swelling, limited use of the prosthetic limb and feelings of instability.

The plan for the surgery is to cut any nerves under a tensioned state and to place the cut away from the bone ends, stitching opposing muscle groups together and underneath the bone ends and to maintain the length of the skin to allow it to fold under the cut end. This way the end of the new extremity can have good skin cover and a suitable cushion of muscle to absorb forces. A typical surgical rule is for every 30 centimetres of a person’s height to give 2.5cm of length in the new limb. A wound dressing is then applied and the person given their post-operative painkillers.

Once the patient has got over their immediate post-operative period they will be seen by the physiotherapist who will check their respiratory status, teach them positional exercises, encourage upper body exercise and assist with safe transfers and early ambulation as appropriate. After perhaps two weeks the physiotherapist may move on to muscle exercises of the extremity itself and may initiate densensitisation of the remaining limb. This reduces the sensitivity of the area so it is prepared for its job with weight bearing inside a prosthesis.

The planning and development for the provision of a prosthesis can be started around the six week point as the operated area will have settled to some degree, remembering that some patients will never be able to use a prosthesis well due to mental inability, overall weakness and poor standing balance. Many complications can potentially interfere with the patient working towards their maximum capacity. Vascular disease means potentially poor wound healing or breakdown and poor skin condition and there may be local swelling, joint stiffness, pain problems and phantom sensations.
